Hospital Coordinator - Full Time

Summit Home Care and Hospice is searching for an experienced and motivated Hospital Coordinator to join our team!

Salary Range: $21.00-$23.00/hour

What the position looks like:

The Hospital coordinator will serve as the conduit to follow patients at a hospital or Skilled Nursing Facility with the goal to return the patient to the care of Summit. The role acts as an intermediary who answers questions and addresses concerns which hospitals, SNF’s, and patients may have during and after receiving care in an effort to continue services with Summit.

What your responsibilities will be:

• Ability to follow current patients that are temporarily domiciled at a Skilled Nursing Facility (SNF) or Hospital.
• Establish relationships with hospital Case Managers to foster a collaborative relationship and increase referrals.
• Educate referral sources on the components of home health care services and explain the interrelation of each of the services to the specific referral sources.
• Assist in identifying and resolving any issue, dissatisfaction, or problem that the referral source is experiencing with the Company’s various services.
• Collaborate with internal partners to ensure unified sales process is understood and adhered to.
• Demonstrate professional conduct and ethics according to organization policies and procedures.
• Able to work cooperatively as a member of a team.
• Ensure that quality and timeliness standards are achieved.
• Identify, track appropriately, and follow current patients to ensure successful resumption of care.
• Utilizes Summit’s EMR and Epic systems effectively.
• Meets established KPI’s.
• All other duties as assigned/directed.

POSITION QUALIFICATIONS:

• The ability to build credibility and trust and collaborate with others.
• Demonstrates interpersonal understanding and utilizes effective communication skills.
• Analyze problems and make decisions in order to deliver results.
• Demonstrates Resiliency and learns continuously.
• Post high school education in healthcare preferred, LPN preferred.
• Minimum 1 year experience in home health, hospice, or other post-acute healthcare area
